To quantify depression symptoms and monitor severity.
Reduce PHQ-9 symptoms (loss of interest, feeling depressed/hopeless, sleep difficulty, low energy, poor appetite/overeating, feeling bad about self or failure, poor concentration, moving slowly or restless, thoughts of suicide or self-harm) from nearly every day.

GOALS
Decrease ACORN symptom for "UNHAPPY" from very often to sometimes, maintain 3 months. Depressed Mood improved from _____ to ____ 1 worst -10 best scale sustained for ___ days, weeks or months Depressed Mood reduced from ___/7 days per week to ____/7 days per week sustained for ___ days, weeks or months Decrease PHQ-9 symptom for "DOWN/DEPRESSED" from very often to sometimes, maintain 3 months. Client will score 20 or below on the Beck Depression Inventory for 5 consecutive sessions. Client's score on the PHQ-9 will be reduced from 20 currently to 10 or less, for 3 months. Client's depression severity will be reduced from 8 currently to 3 or less (based on self-report 1-10 scale), for 3 months. Decrease sadness from daily to 1-2 times per week for 3 weeks. Reduce depression from 6 to a 4 (on a 1-10 scale) sustained for 3 consecutive sessions Reduce depression from 6 to a 4 (on a 1-10 scale) sustained for 3 months Decrease ACORN symptom for "LOW ENERGY" from very often to sometimes, maintain 3 months. Fatigue/tiredness that cause even small tasks to take longer resolved or eliminate, frequency? sustained for ___ days, weeks or months Decrease PHQ-9 symptom for "TIRED/LOW ENERGY" from very often to sometimes, maintain 3 months. Decrease ACORN symptom for "DIFFICULTY GETTING ALONG WITH OTHERS" from very often to sometimes, maintain 3 months.
